
A hospital failed to report to a Tokyo health office a coronavirus infection of a woman in her 50s in August, leaving her to die at home without health condition checks that she meant to have received, the metropolitan government said Monday.
The hospital did not report to the public health center after finding the woman coronavirus positive on Aug. 6, according to the metropolitan government. She died of COVID-19 on Aug. 14.
Although the woman made a phone call to the health center herself, the center failed to monitor her health condition due to the lack of a report from the hospital. Also, the center was unable to work to arrange her hospitalization.
The Tokyo government explained that the hospital mistakenly failed to send the report because it was busy amid the fifth wave of coronavirus infection in Japan this summer.
The Bureau of Social Welfare and Public Health of the Tokyo government said it takes the incident very seriously, pledging to take thorough preventive measures.
